Service Coordination

We can discuss your care needs either over the phone or in person and then connect you with services and resources available in the neighborhood.

Volunteer Coordination

Senior needs may be met by a variety of volunteer sources. Neighborhood and academic service volunteers are committed to providing direct and indirect services to seniors, including:

  • Visits to seniors
  • Respite for caregivers
  • Shopping assistance
  • Office assistance
  • Special project coordination
  • Serving on the board or ad hoc committee
  • Companionship
  • Phone check-ins
  • Seasonal chore help

Faith Community Connection

We work closely with the many faith communities of the Macalester-Groveland neighborhood to provide access to additional services, including:

  • Connection to other community resources
  • Senior home assessments
  • Volunteer recruitment
  • Volunteer training

Support for Caregivers

We also provide supportive services and information to the formal and informal caregivers of seniors in our community.

  • Respite Services: Trained respite volunteers are carefully matched with families who need a temporary break from caregiving.
  • Education and Support: Ongoing education on the specific needs and concerns of caregivers and regular caregiver support meetings are provided.
  • Information and Referral: Find out about other resources available to you as a caregiver.

Home Health Care

In cooperation with Wilder Home Health we coordinate registered home health nursing and home health services.

  • Home health nurses provide assessment and ongoing case management for those who require skilled care.
  • Home health aides provide personal care such as bathing and dressing, some meal preparation and light housekeeping.
